Before any person will be entitled to obtain a license under the provisions of this
article, such person must comply with the following requirements: Submit an application
to be obtained from the city clerk. Such application shall contain therein the name
and place of residence of the applicant together with the name or proposed name of
the business, the proposed business address, the type of business, state license or
certification number and state sales tax number, if applicable. It shall contain any
other relevant data or information as may be needed to properly identify and assess
the proposed business as determined by the city clerk. If, after due consideration,
it is determined that the applicant meets the requirements for the issuance of a city
occupational license, such license shall be issued after the payment of appropriate
fees.
